The structure of a measure details how the measure related to theory and has implications for scoring. Analysis of the structure is a form of construct validity. A measure can be unidimensional, that is all items measure one construct (e.g., Anxiety) and all items reflect a single concept (e.g., general anxiety), or multidimensional, where the Anxiety measure includes items that reflect several aspects of the concept of Anxiety (e.g., social anxiety, post-traumatic stress disorder, general anxiety disorder) which are then scored as subscales.
